Understanding 245D Waivered Services in Minnesota

If you’ve been researching home care options in Minnesota, you’ve likely run into the term 245D. It sounds technical, but the idea behind it is simple: helping people with disabilities and older adults live independently in their own homes and communities. Here’s what it means.

What does “245D” actually refer to?

245D is the chapter of Minnesota law that sets the licensing standards for Home and Community-Based Services (HCBS). These are the supports that allow people to remain in the community instead of institutional settings like nursing homes.

Providers who deliver these services must be licensed under 245D and meet the state’s standards for health, safety, and dignity.

The five 245D “basic support services”

Kalkaal Home Care provides the following 245D basic support services:

1. Individualized Home Supports (IHS)

Support to build independence and life skills at home and in the community — offered with or without formal training goals.

2. Night Supervision

3. Homemaking

Help with cleaning, laundry, meal preparation, and shopping to keep the home safe and comfortable.

4. Respite Care

Short-term relief for family caregivers, giving them a needed break while their loved one is well cared for.

5. Individual Community Living Support (ICLS)

Support for adults 65 and older to live independently — including cueing, supervision, and help with daily tasks.

How are 245D services funded?

245D services are funded through Minnesota’s HCBS waiver programs, such as:

  • CADI (Community Access for Disability Inclusion) Waiver
  • DD (Developmental Disabilities) Waiver
  • BI (Brain Injury) Waiver
  • Elderly Waiver (EW)

To receive services, a person generally needs to be on a waiver and have the service written into their care plan.
