What are 245D services?
Minnesota Statutes chapter 245D sets the licensing standards for Home and Community-Based Services (HCBS) — the supports that help people live in their own homes and communities instead of institutional settings. These services are provided under a person’s HCBS waiver and individualized care plan.
Kalkaal is licensed to provide the full range of 245D services below. Each is delivered by trained staff and coordinated with your case manager.
24-Hour Emergency Assistance
On-call and in-person response to a person’s urgent needs at any hour, so help is always available.
Adult Companion Services
Companionship, supervision, and help with light tasks and errands for adults who need non-medical support.
Homemaker Services
Help with cleaning, laundry, meal preparation, and household tasks to keep the home safe and comfortable.
Night Supervision
Awake overnight staff providing monitoring and assistance in the person’s home.
Respite Care (in-home or out-of-home)
Short-term relief for family caregivers — in the home or another setting — while their loved one is well cared for.
Individual Community Living Support (ICLS)
Support for adults 65+ to live independently, including cueing, supervision, and help with daily tasks.
Individualized Home Supports (IHS)
Support to build independence and life skills at home and in the community.
Semi-Independent Living Skills (SILS)
Training and support that help adults with disabilities gain the skills to live more independently.
