Getting started is easier than you think

The world of home care programs can feel overwhelming — but it doesn’t have to be. We guide you through every step and handle the paperwork, so you can focus on your family.

Step 1

Reach out to us

Call, email, or submit a referral. We’ll listen to your situation and answer your questions — in your language. There’s no cost or obligation to talk with us.

Step 2

Find the right program

We help identify which program fits: PCA, CFSS, or a 245D waiver. If you’re not yet on Medical Assistance or a waiver, we’ll point you in the right direction.

Step 3

Complete your assessment

Minnesota requires an assessment (usually through your county) to determine your services and hours. We guide you through it and coordinate with your case manager.

Step 4

Get matched with a caregiver

We match you with a trained caregiver who fits your needs, schedule, and language — or help you hire an eligible family member or friend.

Step 5

Start receiving care

Your personalized care plan begins. Your coordinator stays in touch to make sure everything is working — and adjusts as your needs change.

How long does it take to start services?

It depends on your assessment and authorization, but we move as quickly as possible. Once your services are approved, we can often begin care within days. Call us and we’ll give you a realistic timeline for your situation.

What if I’m not on Medical Assistance yet?

That’s okay — reach out anyway. We’ll explain how MA and waivers work and help you understand the steps to become eligible.

Do I need a referral from a doctor?

Not necessarily. Many services are accessed through a county assessment rather than a physician referral. We’ll tell you exactly what’s needed for your program.

Can you help in my language?

Very likely. We’re a multilingual team and we arrange interpreter services for many more languages, so you’ll always be understood. You’ll never have to navigate this alone.
